NEW HOPE CHURCH WAIVER & RELEASE

By signing below, the registrant (either an adult on behalf of self, and/or an adult legal guardian on behalf of minor) both agrees and understands the following statements:

I hereby agree to hold New Hope Church, their agents and employees harmless of any and all liability, actions, cause of actions, claims, expenses and damages on account of injury to myself, property, even injury resulting in death, which I now have or which may arise the future connected with this activity or participation in any other associated activities. I also give permission for myself to be transported by the agents and employees of New Hope Church during this Mission. I grant New Hope Church/representative HIPPA release for information required for physician/representative communication and decision making for treatment. I understand that medical treatment may be several minutes away in the case of emergency and transportation options will be made by physician/New Hope staff/representative. Lack of cooperation with policies/rules could result in a registrant returning home at the expense of the registrant.

I understand and will allow photos and videos of myself that are taken while at this New Hope Church activity to be used in any New Hope Church publication. I also understand that publication of these photographs may be accomplished electronically via the internet/Worldwide Web, copying the photographs and video there from, and subsequently using, altering, or republishing them without consent. I waive any claim for damages against the church from un-consented use, alteration, or republication of photographs and video by third parties accessing the internet/Worldwide Web.
